Please ensure that the signed hard copy is submitted and the electronic file is uploaded on time!
(The Word file does not require a signature; the hard copy must be signed. Please type the content for the hard copy—do not handwrite it.)

# For the Major/Minor grading forms: Do not alter the document format, and please read the instructions carefully. Ensure the "Major" field is filled out completely (e.g., "Violin 2"). *You must also complete the "Comments" section; otherwise, you will not receive any feedback!
When uploading your file, please use the following format for both the filename and the email subject line: "Major/Minor Subject - Name" (e.g., "Piano 1 - Wang Xiaoming").
To facilitate the necessary administrative processing, please upload your file and submit your signed hard copy as soon as possible to avoid point deductions. Thank you.
The deadline for late submissions is 12:00 PM (noon) on Wednesday, December 9 (115.12.9). Submissions made after this time will incur point deductions. You must still upload the file and print your own hard copy to provide to the examiners on the day of the exam. Thank you.
Failure to submit by Monday, December 7 (115.12.7), will result in a 3-point deduction from your final practical examination average.
Failure to submit by the late submission deadline of Wednesday, December 9 (115.12.9), will result in a 6-point deduction from your final practical examination average.
